Open Question: im from hawaii, if i am travelling interisland with my child do i need to have her father sign a document ?

Hi im from Hawaii, i have a 3 year old daughter with my ex-boyfriend, and unfortunately i gave his last name to my daughter. Now i am married, and my husband and i are planning to travel only interisland, we’re planning to go to maui with our kids. Do i need to have my ex-boyfriend sign a document in order for us to take my daughter ? I am just wondering, because this is only an interisland travel, which is only a 30 minutes flight. And also how about if we’re planning to go to other states? Do i need a document and have him sign ? Is that how it goes ? Another thing, i wanted to know what kind of documents or papers i need to show at the airport for my kids. I have a 3 years old and an infant. Do i need doctor’s note ? or id’s, which i dont know how that goes cause they are pretty young. hehe. sounds stupid. but ya.. help me ! Anything that they need, such as birth certificate, etc.
